I keep a physical folder labeled “Do Not Repeat.” It’s not sophisticated—it’s printed purchase orders, one page per mistake, with the lesson written in red pen across the top. Embarrassing? Somewhat. Useful? Very. If you total the pages, it comes to roughly $23,000 in wasted budget, and that’s being conservative.
I’ve ordered medical equipment and supplies since 2018, first for a single clinic and now for a five-site health network plus an affiliated dental group. I’m the person who searches “medline equipment for sale,” collects quotes, and signs purchase orders. Along the way I documented nine significant buying mistakes, and the most instructive one was a capnography monitor we bought in September 2022.
The monitor’s sticker price was $3,100. That was a fair price. In fact, it still works fine today. But the purchase ended up costing us closer to $4,700 before it did what our clinicians actually needed. That extra $1,600 wasn’t the monitor’s fault. It was mine—and it’s exactly the kind of cost that doesn’t appear on any quote.
This is the story of what that gap is, where it comes from, and the checklist that stopped me from paying it again.
The surface problem: we compare devices as if price is the whole game
Every serious buyer knows the comparison drill: list competing models, compare clinical specs, compare prices, pick. Search for “capnography monitor” or “blood pressure monitor” and you’ll get a dozen results, from budget models around a few hundred dollars to full multiparameter carts in the five figures. Your brain does what my brain did for years: it anchors on the device. The device is the most visible cost in the purchase, but it’s rarely the largest cost over the device’s life.
Here’s the blind spot I kept hitting: most buyers—and I mean me, for longer than I’d like to admit—focus on the sticker price and miss the accessories, consumables, modules, training, and compatibility costs around it. Those can add 30–50% to what a piece of equipment really costs in year one.

The question everyone asks in purchasing is “What’s your best price?” The question we should have asked before every one of my nine mistakes is “What’s the total cost of making this thing productive in our setting?”
Why the real expense sits outside the box
Once I stopped blaming vendors—and my own spreadsheet—and actually reviewed the paperwork in the folder, a clear pattern emerged. My mistakes weren’t random. They came from three assumptions that turned out to be wrong.
Assumption one: you’re buying a device. In reality, you’re joining a system. Every monitor uses cuffs, sensors, sampling lines, and connectors, and many of those are proprietary or partially proprietary to the manufacturer. What I mean is the hardware is the front door. The consumables are the rent, and the rent comes due every month. Capnography is the clearest example. Some monitors use mainstream technology, which measures CO₂ through an adapter in an intubated patient’s breathing circuit. Others use sidestream technology, which can sample from a small nasal cannula—essential for patients who are sedated but breathing on their own. If you buy the wrong technology for your clinical setting, no amount of negotiating on the monitor price fixes it. The device works; it just doesn’t work for your patients.
Assumption two: “compatible” and “standard” mean one thing. They don’t. In my first year, I learned that “standard” blood pressure cuffs meant something different to each manufacturer in our building. Even within one brand, a new monitor generation can switch connectors and make your inventory obsolete. This was true a decade ago, and it’s still true today. The “everything is interchangeable” thinking comes from an era when clinical equipment was simpler and largely analog. That changed.
Assumption three: the number we optimized was the right number. Procurement is trained to minimize the cost per unit. But unit price is only half of the equation. The other half is how fast you use the item, how it’s stored, whether staff can operate it competently, and what happens when it fails. A saline bag priced lower per liter isn’t a bargain if your clinical areas genuinely use 500 mL bags and waste the rest. An autoclave priced well isn’t a bargain if nobody on the team has been trained to operate it correctly. More often than not, what looked like a purchasing problem was an operational problem wearing a price tag.
What my mistakes actually cost—itemized
Numbers make this real. Here are the examples from my folder that hurt the most.
The capnography mismatch (September 2022). Our procedure clinic needed capnography for non-intubated, sedated patients. The $3,100 monitor I chose was configured for mainstream technology, which suited intubated patients but not our nasal-cannula population. Not the vendor’s mistake—mine. I’d compared screen size, battery life, and price, but I didn’t know to ask about sampling technology. Fixing it required a sidestream retrofit module at $890, an installation and calibration visit at $320, and roughly $390 in sampling supplies we bought before we understood the setup. Add the three weeks of rescheduled procedures and the trust we lost with the clinical team. That’s the $1,600 of pain I mentioned at the start.
The drawer of blood pressure cuffs (2018). In my first year, I ordered 40 “compatible” adult cuffs to standardize two clinic sites, and I didn’t check the connector type on either site’s monitors. About 30 of those cuffs didn’t fit at all; the remaining 10 connected fine but triggered “check cuff” errors because the pins didn’t match. Almost $800 of that order went into a drawer labeled “Do Not Open.” The mistake wasn’t the cuff price—it was that I optimized for the cuff price alone.
The saline volume error (2021). Medline sterile 0.9 normal saline is a perfectly standard product; the error was mine, and it was about sizing. I bought a large quantity of 1-liter bags because the per-milliliter cost was attractive. Our infusion area mostly uses 500 mL bags for these procedures, and a partially used bag can’t be saved for the next patient. We generated waste on nearly every use, and the overstock lingered past its shelf life in storage. The unit price was lower. The total cost was higher.
The dental autoclave gap (March 2023). I bought a capable autoclave for our dental group and treated training as optional. Before anyone flagged it, a newer staff member had been running it with tap water and overpacked pouches, which caused scale damage and a $420 service call—and, far worse, a weekend of reprocessing instruments used in six patient procedures. Nothing harmed, but plenty learned. - Describe the clinical setting first—intubated or not, acute or long-term, procedure volume, and who will operate it. The right device for one setting can be the wrong device for another.
- List what the quote excludes: modules, mounts, cuffs, sensors, cannulas, and installation. If it isn’t in the quote, it isn’t free.
- Price the consumables per year—not per box. For a monitor, cost per procedure matters most.
- Verify compatibility against your actual inventory, not just against the word “standard.” Call the manufacturer with your existing model numbers if needed.
- Book training before delivery. If the vendor can’t provide in-service instruction, that capability gap is a cost.
- Check shelf life and real usage rate before accepting volume discounts. A lower unit price only helps if it doesn’t create waste.
Five minutes of verification beats five days of correction. I know, because I paid for the correction before I started using this checklist. The mistakes in my folder are part of my job now: they trained me, and now they’re training my team.
Your equipment budget deserves the same respect. Compare systems, not just prices. And write down your errors before they repeat themselves.
